Opioid use disorder and chronic pain – Judicious use of opiods with Léonie Archambault and Laurent Dupuis
Chronic pain and opioid use disorders (OUD) commonly co-occur. In fact, among people treated for OUD, it is estimated that 55 to 62% experience chronic pain. Conversely, among people receiving long-term prescribed opioids for pain, 8 to 12% may develop an OUD. Bidirectional processes may be involved in the development of this dual issue, such as self-medication for physical or psychological suffering, iatrogenic dependence, risk-taking, or accidents. The biopsychosocial nature of the co-occurring conditions requires a multimodal, interdisciplinary therapeutic approach. This presentation aims to briefly describe :
• Links between chronic pain and opioid use disorder (OUD)
• A “chronic pain profile” in a sample of patients admitted for OUD treatment
• Service experiences for patients with chronic pain and OUD
• Avenues for service improvement and judicious opioid use
Quebec clinical guide on support for people living with opioid use disorder: At the crossroads of scientific, clinical and experiential knowledge with Marie-Ève Goyer
The mandate of the Addiction and homelessness clinical and organizational support team of the CIUSSS Centre-Sud-de-l’Île-de-Montréal is to support clinicians and managers in the health and social services network working with people living with addiction or homelessness. To this end, the team carries out a variety of activities, including the creation of tools and guides, knowledge transfer and clinical coaching. In 2024, ESCODI published the first Quebec guide to support health care providers that treat people living with an opioid use disorder. This guide aims to develop a holistic vision of the approach to people struggling with addiction by supporting clinical approaches, but also by recalling the fundamental values of the intervention philosophy. At the same time, a guide for people in treatment has also been produced, entitled: Opioid dependence: my treatment, my choices. Finally, the team also conducted a scoping study of the evidence base for trauma- and violence-informed interventions in addiction and homelessness, which paved the way for the launch of Quebec’s first guide on the subject, due in May 2025.
